Community Health Collaborative

REACH CNY and Oswego County Opportunities (OCO) OPTIONS Program partner in the Perinatal and Infant Community Health Collaborative (PICHC) serving Oswego County. 

PICHC strives to improve overall health of birthing individuals, their infants and families and improve overall health outcomes.  The PICHC Program works together with community members and other organizations in Oswego County to reduce racial, ethnic, and economic disparities in health outcomes and address the factors that affect those disparities.  We focus on building systems of care for birthing individuals as well as offering individual level care to those in need through the Community Health Worker (CHW) Program at OCO OPTIONS. 

CHWs offer home visits to assist individuals who are pregnant or parenting children under age 2 access and navigate healthcare and other essential resources and support services.  They work with individuals to create person-centered goals, and offer 1:1 education on a variety of different health related topics, child birth education, breastfeeding support, smoking cessation counseling, Circle of Security Parenting series, and other supportive services.

Community Health Worker Referrals

Anyone can refer an individual for services – a doctor, nurse, agency or school employee, family member. Individuals can even self –refer.

Health Insurance

PICHC strives to find and engage high-need women and their families in health insurance.  Based on individual income, financial assistance in obtaining health insurance is available through NY State of Health.  

NY State of Health is an organized marketplace designed to help people shop for and enroll in health insurance coverage.  Individuals, families and small businesses can use the Marketplace to help them compare insurance options, calculate costs and select coverage.  The Marketplace uses a single application that helps people to check their eligibility for health care programs like Medicaid, Child Health Plus, and the Essential Plan and enroll in these programs if they are eligible.

Safe Sleep Program

REACH CNY is a Cribs For Kids® Affiliate. We partner with Cribs for Kids, New York State Infant Mortality Reduction CoIIN initiative, Oswego County Health Department, OCO OPTIONS and many other programs to bring safe sleep education to providers and clients and provide portable cribs to parents in need.
