Maternal & Infant Community Health Collaborative Oswego County
REACH CNY and Oswego County Opportunities (OPTIONS program) are partnering to bring the Maternal and Infant Community Health Collaborative (MICHC) to Oswego County. At MICHC, we want women to achieve their best health before pregnancy, during pregnancy and after pregnancy. We focus on building systems of care for women as well as offering individual level care to those who need it. Community Health Workers help women access and navigate healthcare and other essential support services, to provide individual education through home visits, breastfeeding support, and other services.
MICHC strives to find and engage high-need women and their families in health insurance. Based on individual income, financial assistance in obtaining health insurance is available through NY State of Health.
NY State of Health is an organized marketplace designed to help people shop for and enroll in health insurance coverage. Individuals, families and small businesses can use the Marketplace to help them compare insurance options, calculate costs and select coverage. The Marketplace uses a single application that helps people to check their eligibility for health care programs like Medicaid, Child Health Plus, and the Essential Plan and enroll in these programs if they are eligible.

Family Planning Benefit Program
FPBP is a public health insurance program for New Yorkers who need family planning services, but may not be able to afford them. It is intended to increase access to confidential family planning services and to enable teens, women and men of childbearing age to prevent and/or reduce the incidence of unintentional pregnancies. Safe Sleep Program
REACH CNY is a Cribs For Kids® Affiliate. We partner with Cribs for Kids, New York State Infant Mortality Reduction CoIIN initiative, Oswego County Health Department, Catholic Charities, OCO OPTIONS and many other programs to being safe sleep education to providers and clients and provide portable cribs to parents in need. Community Health Worker Referrals
Anyone can refer an individual for services – a doctor, nurse, agency or school employee, family member. Clients can even self –refer. Please call OPTIONS OCO at (315) 342-7532 ext. 5 or Fax Referral form to (315) 342-7554.
